Fluoride works differently. It doesn’t replace lost mineral directly the way hydroxyapatite does. Instead, it integrates into the enamel structure during remineralization and makes the resulting enamel more resistant to future acid attacks than it would otherwise be.
The research gap, honestly stated
Fluoride has been studied for over 70 years, across huge populations, with strong evidence for cavity prevention. That’s not a close comparison in terms of sheer volume of data, and I’m not going to pretend it is.
Hydroxyapatite’s research base is smaller but not nothing. Japan has used it in toothpaste since the 1980s, and more recent European studies, including some head-to-head trials against fluoride, have found nano-hydroxyapatite performs comparably to fluoride for remineralization and cavity prevention in the populations studied. “Comparably” is doing real work in that sentence — it’s not “proven superior,” it’s “held its own in early direct comparisons,” which is a meaningfully different claim than the marketing around hydroxyapatite toothpaste usually makes.
Why this matters beyond toothpaste aisle marketing
